Most of them do yeah, I've known a few people diagnosed with personality disorders and they know they are nuts but most of the time they have what's called a narcissistic shell which is like this bubble of dilusion they exist in where they convince themselves they are normal. Every now and again reality gets through though, if you get them in a vulnerable moment they will admit they are fooked in the head.
